PROCEDURE
Scaffolds shall be provided for all work that cannot be done safely by employees standing on permanent or solid construction at least 20 inches wide, except where such work can be safely done from ladders. The following procedures apply to all scaffolds for Replace with Company Name.
PROHIBITED TYPES OF SCAFFOLD
There are many unfortunately all too common types of scaffolds that are too unsafe to use. These include, but may not be limited to: lean-to or jack scaffolds, shore scaffolds, nailed brackets, loose tile, loose brick, loose blocks, stilts, or other similar unstable objects shall not be used as working platforms, or for the support of such platforms. If in doubt, check with your supervisor or RSO for approved types of scaffold. Job-built wooden scaffolds constructed of dimensioned lumber are not permitted. Scaffolds shall be constructed of suitable materials such as steel or aluminum members of known strength characteristics. Where materials other than wood are used, or where scaffold designs differ from those specified in these Orders, the scaffold and its parts must provide a degree of strength, rigidity and safety equivalent to that provided by the described scaffold it replaces.
TRAINING
Replace with Company Name is required to train all employees regarding hazards by "qualified" persons. “Qualified” in this context means a person with a recognized certification as having specialized knowledge in scaffold erection. dismantling, and use (for instance a factory representative. The training program must address three hazards (fall, electrical, falling objects), personnel fall protection, scaffold use and load capacity. Re-training is required when a lack of proficiency is noted, or a new hazard or scaffold type is introduced into the workplace for which the employee was not trained.
INSPECTIONS
A competent person must direct the erection, alteration, and dismantling of all scaffold. In addition, a competent person must make an inspection of the entire scaffold assembly at the start of each workshift to ensure that scaffolds are safe prior to and during use. The competent person shall determine if additional inspections are necessary during the workshift. After an inspection, if any defects are noticed, the competent person shall tag the scaffold “Do Not Use” and disallow use of the scaffold until the defect(s) has been corrected. No employee shall use any scaffold that is tagged out of service.
