An excerpt from ISNetworld® RAVS®* Safety plan Ladder Safety Program

It is the policy of Replace with Company Name to take every reasonable precaution in the performance of work to use ladders properly at all times.

Ladders must be in good condition, made of suitable material, of proper length, and of the correct type for the use intended – that is ladders shall be used only for the purpose for which they are designed. Ladders used by our employees must meet OSHA/ANSI specifications. Ladders shall be used only for the purpose for which they were designed. Never use ladder in a horizontal position or as scaffolding, do not place ladders on top of boxes, barrels, crates, etc. Ladders shall not be altered, modified, or attached to other ladders to extend their reach. Ladder rungs, cleats, and steps shall be parallel, level, and uniformly spaced, when the ladder is in position for use.

Damaged ladders must never be used; they should be repaired by a qualified person tagged or destroyed. Portable ladders with structural defects, such as, but not limited to:

Broken or missing rungs, cleats, or steps, broken or split rails
Corroded components, or other faulty or defective components,
Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components

Portable and fixed ladders with defects shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired, or blocked (such as with a plywood attachment that spans several rungs).
